Case Manager
Position Summary: The Case Manager is responsible for working directly with Victims of sex trafficking and sexual exploitation. We’re looking for inspiring team members who enjoy bringing out the best in people and are especially motivated to work and serve.
As a Case Manager, you will:
Help create a supportive, homelike environment for the individuals in our residence; assist program participants in making plans to meet their specific recovery goals; restore and teach daily living skills; and engage participants in meaningful activity in the house and in the community; write daily shift notes and other communications.
The duties shall include, but not be limited to the following:
- Providing information and referral services for people reaching out through the Fields of Hope outreach line.
- Provide crisis intervention and case management services for onboarding new clients and those already in community-based services.
- Provide trauma-informed survivor advocacy services and care to clients in residential program on a part-time weekly basis.
- Provide support toward goal attainment for each client according to individualized education and case management plan.
- Facilitate access to medical, mental health, therapeutic recreation and alternative therapies (equine-assisted, art, horticulture, other) services provided through Fields of Hope, partners and community agencies.
- Create, update and maintain client files, and provide monthly reports to supervisors.
- Follow guidelines outlined in the Fields of Hope P&P Manual.
- Participate in all team meetings as required.
- Other duties as assigned.
Minimum Education and Experience – Bachelor’s Degree in Social Work or related field with 2-5 years of experience. Prefer experience with vulnerable populations, trafficking, and previous residential program experience.
Additional Minimum Needs:
- Experience in Counseling.
- Administrative skills (typing, emailing, faxing, answering phones, using a printer).
- Familiarity with Microsoft Office functions – Word, Excel, Power Point, etc.
- Leadership skills, with ability to respect and submit to those in authority.
- The ability to be a good manager of time and resources, and practice good self-care.
- Ability to tolerate conflict and effectively manage it; maintaining confidentiality.
- Consistency and a non-judgmental attitude.
- Tact (slow to anger, and not reacting to situations without thinking before speaking).
EDUCATION:
- High School Diploma, or GED is required.
Requirements
- Certification in CPR and first aid
- Valid driver's license and reliable transportation
- Clear criminal background check
