POSITION PURPOSE:
This position is responsible for providing case management services to patients participating in mental health services at Fillmore County Hospital Integrated Health. The Case Manager’s activities may include pre-admission and pre-discharge planning, services coordination and delivery, assistance in accessing basic needs, medical, psychiatric, social, educational, financial, and vocational services.
QUALIFICATIONS:
- Bachelor's degree in Social Work or Human Relations or equivalent experience required
- Valid Nebraska driver’s license
- Clear driving record
- Basic Life Support (BLS)
- ACLS preferred
GENERAL REQUIREMENTS:

SECURITY/ACCESS:

Essential Job Duties and Responsibilities:
1.
- Establishes a therapeutic relationship with assigned patients.
- Understands and implements individual treatment plans.
- Helps patients with daily living.
- Records and reports patient behavior to appropriate clinicians/staff.
- Provides a safe and supportive environment for patients.
2.
- Transfers patients in a safe manner, demonstrating proper uses of a gait belt and Sit and Stand Lift.
- Assists with the patient’s personnel hygiene as the condition warrants, including toileting and hand washing/use of alcohol gel.
- Serves as an advocate for the patient
3.
- Prepares and assembles newly admitted patient medical records.
- Completes patient registration and schedules patient appointments.
- Conducts insurance verification and eligibility checks.
- Assure correctness of all paperwork placed in charts.
- Completes chart audits on new patients as directed.
- Completes discharge charts within 30 days of dismissal.
- Completes vehicle inspections and sends in weekly/monthly.
- Reviews daily schedule to ensure patient needs are met.
- Maintains detailed records, appropriate documentation logs and program tracking information.
4. Linking the individual directly to services and supports specified in the individual service plan or by therapist referral. Assisting the individual directly for the purpose of locating, obtaining and effectively using community resources.
- Working collaboratively with clients’ care managers and other care providers
- Maintain a current awareness and partnership with community resources.
- Maintain ongoing contact with identified patients to provide wraparound support.
- Attend team meetings, other professional meetings and represent the program to the community as needed.
- Coordinating services with other agencies and providers involved with the individual (e.g., DHHS, Area Agency on Aging, Social Security Administration; this also includes assisting clients or their family members to apply for benefits, which they are eligible to receive
5. As a back up:
- Provides safe and quality transportation services in a manner that complies with all local, state and federal laws
- Verifies van schedule at the end of each day for the next day and again first thing in the morning.
- Maintains complete and accurate daily records on the transportation activity, passengers, and mileage.
- Gives the best possible first impression by greeting all passengers with a warm, sincere, and friendly welcome.
- Ensure that passengers are transported in a safe, comfortable, and timely manner.
- Assists each passenger in and out of the van, ensure that they are secured safely in their seats or wheelchair, and help them in and out of their home/destination.
- Contact patients to confirm their attendance if any questions so as to alert transportation if to make the trip.
- Maintains van in good working condition at all times with on-going preventative maintenance.
- Completes daily and weekly safety inspections on the van.
- Keeps the van clean and attractive in appearance at all times.
- Stores van appropriately and securely at the end of shift.
6. Follows HIPAA regulations and ensures that the confidentiality of patients medical, personal, financial records, employee records, and organizational records is maintained.
7. Participates in FCH committees, quality assurance, performance and quality improvement initiatives, and activities which support the facility and department operations.
8. Complies with the state and federal regulatory requirements related to the performance of FCH operations and requirements of the FCH Compliance Program. Including but not limited to:
- The Compliance Policy.
- Completes all mandatory on-line training annually and as assigned.
- All department and organization-wide policies.
- Considers the impact of regulations such as Critical Access Hospital requirements, along with Federal and State of Nebraska regulations and discusses concerns appropriately to ensure compliance.
9. Presents self in a professional manner and enhances professional growth and development through participation in education programs, current literature, in-service meetings and workshops.
