Job Title : Targeted Case Manager – Certified Community Behavioral Health Center (CCBHC)

Schedule : Monday - Friday, 8 : 30a-4p.
Job Summary :
The Targeted Case Manager for the Certified Community Behavioral Health Center (CCBHC) plays a key role in improving client health outcomes by addressing behavioral health, mental health, and social determinant needs. This position supports clients by coordinating clinical and non-clinical services and helping them navigate these systems effectively. The CCBHC employs a high-quality, consistent set of protocols for screening, intake, outreach, navigation, and care coordination, ensuring clients receive the necessary resources to meet their recovery goals.
Required Qualifications :
- High School Diploma or GED required.
- Minimum 1 year of related experience, preferably in Behavioral Health.
- Proven ability to establish supportive client relationships.
- Ability to perform duties in a stable, consistent, and predictable manner.
- Valid NYS Driver’s License in good standing.
