As the Public Health Support Specialist you will…
· Consult with and educate families regarding the services available through the Care For Kids program 1st Five programs and Firefly.
· Support other Public Health department functions, including providing and documenting screenings as needed.
· Actively participate in a multidisciplinary team to promote interagency collaboration in planning, implementation, and continuous quality improvement.
· Meet all documentation and productivity standards as identified by the agency. Ensure the completion of case documentation, as required by agency and grant sources. Maintain confidentiality of all families and team members.
· Seek opportunities to enhance one’s own professional knowledge, skills, and abilities as they relate to one’s current position and/or to prepare for potential future roles and overall career development
· Function independently with minimal supervision
· Perform other duties and responsibilities as requested.
As the Public Health Support Specialist (63%), you will…
· Ensure care coordination services for all Medicaid-enrolled children, including assisting families in finding a local dentist and/or medical provider.
As the Developmental Support Specialist (37%), you will…
· Ensure coordination services for all referred children birth to age 5, including but not limited to:,
Serve as a resource for parents and caregivers
Conduct screening and developmental monitoring as appropriate
o Assist families in finding a local dentist and/or medical provider
o Assist families in setting up and keeping appointments when needed (including transportation)
o Develop referral/recall system
o Provide anticipatory guidance
o Assist families in finding payment sources for care
You’ll need to have…
· An understanding and commitment to the agency's mission.
· 4-year degree in public health, social work, human services, early childhood education, nursing, or related field; be a Registered Nurse (RN); or be a Licensed Practical Nurse (LPN)
· Ability to work at a desk on the computer and phone for the full shift.
· The ability to communicate appropriately verbally and in writing with clients, colleagues, referral sources, funding sources, and other community resources.
· Respect for and sensitivity to the needs and rights of others, including those with differing cultural, racial, sexual, or ethnic identities.
· The ability to work flexible hours, which include weekends and/or evening hours with notice
· The ability to pass background checks, drug tests, and any other examination required by federal law and/or the State of Iowa.
Bonus points if you have…
- Bilingual (English and Spanish) written and oral proficiency.
- Experience working in the public health sector with a focus on families and/or children.
- Knowledge of community resources.
