Description
The Help Me Grow (HMG) Community Liaison is responsible for developing, establishing and monitoring relationships with and between child-serving agencies as part of the Pediatric Care Coordination (PCC)/HMG System of Care to create and promote an inventory of available child development services, foster interagency relationships and networking, promote the use of the HMG system, and identify service gaps and barriers.
- Implements outreach strategies, and develops collaborative relationships with networks, including providers, agencies and organizations that provide resources, information and referral services to children/families with developmental, behavioral and/or learning concerns.
- Identifies and builds relationships with child-serving agencies that provide direct services to families to ensure their timely and accurate participation in the development and maintenance of the Resource Database
- Creates and maintains a tracking system to ensure regular contact with identified agencies to promote interagency relationships and support their participation in the resource database
- Works closely with First 5 Alameda County (Agency) and partner staff assigned to the Resource Database to ensure that the database is accurate and up to date
- Plans and provides trainings/presentations about Help Me Grow, its various components, including the provider database, and the use of the Help Me Grow system
- In collaboration with the Help Me Grow Leadership, develops and implements Connection Cafés and plans, facilitates, and hosts provider networking meetings
- Collaborates in the planning and implementation of community events to promote child development and Help Me Grow; helps identify appropriate events to participate in; staffs and plans event logistics (includes weekend events)
- Works closely with the Help Me Grow Leadership to identify content and develop social media campaigns that help achieve program goals
- Collaborates in website strategy and management
- Oversees an electronic listserv and identifies up-to-date information about resources, events, and special needs of families, etc. for mass distribution
- Collaborates with and provide specialized case support to the HMG team that provides phone line and direct linkage support
- Analyzes available data and collaborates with community networks, HMG Centralized Access Point, and Family Navigators to identify resources and gaps in services; and makes recommendations to appropriate First 5 staff, external staff and agencies, funders, policy makers and others to address these gaps
- Participates as a member of the HMG outreach team and collaborates with other team members in conducting outreach activities
- Supports the integration of CalAIM, California’s Medicaid transformation, by collaborating with cross-functional teams to streamline access to healthcare services and enhance care coordination.
- Participates in various collaboratives and coalitions as appropriate
- Regularly drives and/or travels throughout Alameda County to perform the above job duties; driving is an essential function of this position
Qualifications
Minimum Qualifications
Any combination of training and experience that would provide the required knowledge, skills, and abilities is qualifying. A typical way to obtain the required qualifications would be:
- A minimum of seven years of full-time progressively responsible experience in a public or private organization serving at-risk families with young children including some direct service experience and the responsibility for program management, planning and/or evaluation.
- Associate’s degree or equivalent of two years of college (90 quarter units or 60 semester units) from an accredited college or university may be substituted for 2 years’ required experience. Bachelor’s degree in social work, public health, social services, or related field may be substituted for an additional 2 years required experience.
Other
Travel within Alameda County as necessary to carry out job duties. Some travel outside Alameda County occasionally required.
Knowledge of
- Principles and practices of child development and early childhood mental health including early identification and screening practices and tools
- Strategies for working effectively with at-risk families
- Principles of family-centered and family-driven care
- Early childhood and early intervention systems and services; prefer knowledge of Alameda County services
- Program development, planning and evaluation methodologies
- Principles and practices of quality assurance
- Fluency with social media management, messaging, branding and content strategy
- Basic digital communications across platforms, content creation across platforms, and data analysis
- Basic budgetary and financial recordkeeping procedures
- Proficiency in Microsoft Office Suite

- Meet attendance requirements of the position, be punctual and timely in meeting all requirements for work performance

- We are currently working a hybrid 2 day per week in-office schedule and employees also come to the office to attend meetings and as requested.
- All First 5 employees must live and work in California.
- The salary range for this position is $75,000 to $102,000 and as an independent government agency, the posted salary is the set salary range for this position. We cannot offer salaries above this posted range.

Salary : $75,000 - $102,000
