We are seeking a compassionate and dedicated social worker to join our team at First Alliance Healthcare of Ohio. As a social worker, you will be responsible for providing direct support and advocacy to individuals and families in need. Your role will involve assessing clients' needs, developing care plans, coordinating services, and connecting clients with community resources. The ideal candidate will have a strong commitment to social justice, excellent communication skills, and the ability to work collaboratively in a multidisciplinary team.

Responsibilities :

• Conduct initial assessments to gather comprehensive client histories, strengths, needs, and goals.
• Coordinate the intake process by evaluating eligibility for services and connecting clients with appropriate resources.
• Collaborate with clients, families, and interdisciplinary teams to develop individualized care plans.
• Provide support and guidance to clients as they navigate mental health services.
• Advocate for clients' access to necessary services, ensuring a smooth transition into care.
• Maintain accurate, confidential, and timely documentation of all client interactions and assessments.
