Requirements :

Covid Vaccine required.

Minimum of 1 year of experience.

BLS / CPR Certification.

Current PPD (within 1 year).

Active LPN license.

Must be able to lift at least 50 pounds without restrictions.

Able to stand and walk for extended periods.

Ability to bend and lift without restrictions.

Job Description : As an LPN Care Coordinator, your role is to provide thoughtful and professional care for patients. We are looking for qualified and compassionate candidates who thrive in a fast-paced environment. This position will require physical endurance and mental acuity, but it is incredibly rewarding.

Key Responsibilities :

Delegate tasks to CNAs to ensure patients receive the highest level of care.

Administer medications and treatments according to MAR and TAR following physician orders.

Perform wound care as prescribed by the physician.

Manage patient admissions and discharges according to facility protocols.

Conduct ongoing patient assessments in accordance with physician orders.

Educate patients and their families about medications and treatments, respecting confidentiality under HIPAA.

Familiarity with G-tubes, nebulizer treatments, and comfort with tracheostomy care is essential.

Skills Needed :

Experience as a CNA or LPN.

Ability to handle diverse care tasks and work effectively within a multidisciplinary team.

Knowledge of hospital health, safety, and sanitation standards.

Understanding of confidentiality obligations and nursing best practices.

Strong medical and hospital terminology knowledge.

Proficiency in MS Office and data entry.

Excellent physical condition and stamina.

Strong communication and interpersonal abilities.

Compassionate nature and ability to handle stressful situations.

Successful completion of an accredited LPN or CNA program is a must.
