We are currently seeking a licensed Nurse Practitioner to join our team. This role involves working across diverse settings, including clinics, skilled nursing facilities, home health care, and more.
Job Responsibilities
· Deliver care across various settings, including clinics, skilled nursing facilities, home health, and more.
· Assess and manage acute, chronic, surgical, and dermatologic wounds.
· Perform dressing changes and medically necessary debridement procedures.
· Develop individualized care plans and adhere to protocols established by the medical director.
· Conduct thorough physical examinations for adult patients with acute and chronic wounds.
· Diagnose, assess, and manage chronic conditions within the patient population.
· Communicate with the patient’s Primary Care Physician (PCP) or office staff regarding any significant changes in the patient’s condition.
· Provide tailored patient education, teaching, and counseling.
Qualifications:
• Current Nurse Practitioner license.
• Certification as a Wound Care Nurse Practitioner preferred.
• Strong communication and interpersonal skills.
• Compassion for patients and a commitment to providing excellent care.
