A Licensed Practical Nurse (LPN) in Long-Term Care (LTC) in Oswego, NY, is responsible for providing compassionate nursing care to residents in a long-term care facility. Key responsibilities include :
Assisting residents with daily activities such as bathing, dressing, feeding, and mobility support.
Administering prescribed medications and treatments, while monitoring for side effects or complications.
Performing routine nursing assessments, documenting patient progress, and updating care plans as necessary.
Monitoring and recording vital signs, and reporting any changes in condition to the supervising Registered Nurse (RN) or physician.
Collaborating with the interdisciplinary care team to provide holistic care for each resident.
Responsibilities
Provide compassionate and high-quality care to residents in a Long-Term Care (LTC) setting while ensuring their safety and comfort.
Administer medications accurately as per physician orders and monitor for any adverse reactions.
Assess and treat wounds, pressure ulcers, and other skin conditions following facility protocols.
Regularly assess and document patient vitals, including blood pressure, heart rate, temperature, and oxygen levels.
EMR Documentation : Proficient in electronic medical records (EMR) systems to maintain accurate and timely patient records.
Work closely with RNs, CNAs, and other healthcare professionals to provide comprehensive patient care.
Communicate clearly with residents, families, and the healthcare team to ensure continuity of care.
Advocate for residents' health, comfort, and emotional well-being.
Efficiently manage multiple responsibilities in a fast-paced environment while prioritizing patient needs.
Adhere to infection prevention protocols, safety regulations, and facility policies.
Requirements
Active New York State (NYS) LPN License (Required)
BLS from AHA Required
Minimum 2 years of experience in a Long-Term Care (LTC) setting
Must be willing to float between units as needed
Ability to work 2 : 00 PM - 10 : 30 PM shift (Including every other weekend)
Strong clinical skills in patient care, medication administration, and wound care
Proficiency in EMR systems for accurate documentation
Must confirm full commitment to the contract upon acceptance
