Job Description
COVID-19 vaccination is recommended but not required. Flu vaccination will be required October through April. Flu exemption applications must be received and approved by Phoenix Childrens prior to starting. Anyone not vaccinated for either Flu or COVID-19 will be required to sign Declination & Masking Protocol agreements and must mask during the full respiratory virus season.
This assignment will be at our Avondale Campus, 1665 N. Avondale Blvd., Avondale, Arizona 85392.
Job Summary
This position provides occupational therapy services to a pediatric population, neonate to adolescent, including evaluation and treatment in independent living, sensory-motor integration, cognitive and psychosocial skill.

Experience Requirements
- 1 year Pediatric work experience (school experience will not be considered)
- 1 year outpatient experience.
- Certification with the National Board for Certification in Occupational Therapy (NBCOT) prior to start date
- State of Arizona Occupational Therapist license prior to start date
- Current AZ Department of Public Safety Fingerprint Clearance Card prior to start date
- Current BLS certification from the American Heart Association prior to start date
