CCM (Certified Case Manager), ACM (Accredited Case Manager), C-SWCM (Certified Social Work Case Manager), CBHCM (Certified Behavioral Health Case Manager)": Not required
Case load/# patients per day/consults per day: 15-20, Varies based on need
EMR: Meditech
Do you require InterQual criteria or Milliman experience: No
Is this position for UR, discharge planning, or both? DC planning and clinical social work
What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.): Inpatient and ER
Common types of patients & job duties: care management for transition of care planning including home, HH, SNF, IRU, LTAC, hospice, etc. Social work psychosocial assessments for safety planning on inpatient psych, assist with difficult placement and bio-psychosocial needs of patients, provide resources and collaboration with community partners to connect patients with needed services
Experience or 'Must have' skills: Clinical Social Work License in state of Oregon, and Acute hospital experience
CM Manager/Supervisor: No
Shifts & Scheduling? Monday - Friday 8-4:30
Weekend Requirement: Possibly as needed
Holiday Requirements: Rotation, depends on coverage
Scrub Color/Dress Code: Business casual
RT for DC Planning? No
Pharmacy for DC Planning? No
