The Program Manager for Tiny Home Midvale is responsible for overseeing the daily operations of the Tiny Home Village, ensuring a safe, low-barrier, trauma-informed environment for individuals transitioning from homelessness to permanent housing. This role involves direct case management of up to 8 clients while collaborating closely with County District representatives, LAHSA, and PATH outreach teams to coordinate services, facilitate housing navigation, and strengthen program efficiency.
This position ensures compliance with LAHSA’s Scope of Required Services (SRS) and other funder guidelines while providing individualized support to clients in their journey toward permanent housing stability.
Program Oversight and Compliance
- Manage daily operations of the Tiny Home Village ensuring adherence to LAHSA, County, and CES (Coordinated Entry System) requirements.
- Oversee a low-barrier, trauma-informed environment where participants have access to 24/7 shelter services, case management, and housing navigation support.
- Maintain program compliance with funding requirements, safety protocols, and participant eligibility criteria.
- Work alongside LAHSA, County District representatives, and PATH outreach teams to ensure appropriate encampment-to-shelter referrals and intake processes.
- Oversee residential supervision and site security, ensuring safety and adherence to harm reduction practices.
Case Management and Participant Support
- Carry a caseload of 8 clients, providing housing-focused case management, supportive services, and problem-solving strategies.
- Conduct intake assessments and develop individualized Housing Services Plans to ensure successful transitions into permanent housing.
- Facilitate referrals to housing navigation programs, employment services, behavioral health providers, and other supportive services.
- Ensure all clients have access to mainstream benefits, mental health services, medical care, and substance use treatment as needed.
- Maintain a client-centered approach with a strong emphasis on Housing First, Harm Reduction, and Trauma-Informed Care principles.
- Coordinate with PATH outreach teams and LAHSA representatives to ensure clients have document-ready status for housing placements.
Collaboration and Stakeholder Engagement
- Work side-by-side with County District representatives, LAHSA, and PATH outreach teams to address homeless encampment resolutions and enhance rapid rehousing efforts.
- Attend regular inter-agency meetings and regional coordination discussions to align services and share best practices.
- Serve as a liaison between Tiny Home Midvale, local service providers, and community partners to expand resources for clients.
- Advocate for policy changes and additional funding opportunities to enhance the impact of the Tiny Home program.
Data Management and Reporting
- Ensure accurate and timely HMIS data entry with a minimum of 95% data quality compliance.
- Document all participant interactions, case notes, and referrals in HMIS in real-time or within 24 hours of service delivery.
- Track Key Performance Indicators (KPIs), including participant retention rates, average length of stay, and housing placement success.
- Prepare monthly and quarterly reports for LAHSA, County, and internal stakeholders regarding program outcomes and client progress.
Staff Supervision and Training
- Supervise and support program staff, ensuring adherence to case management best practices and client engagement strategies.
- Conduct weekly case conferencing with staff to review client progress, barriers, and service gaps.
- Ensure all staff and volunteers are trained in Trauma-Informed Care, Harm Reduction, and Housing First methodologies.
Facilitate de-escalation and crisis intervention training for staff to ensure client safety and program compliance.
- Support re-entry efforts for individuals exiting institutions such as hospitals, jails, and treatment centers by facilitating connections to housing and wraparound services.
Job Type: Full-time
Pay: $80,000.00 - $90,000.00 per year
