What are the responsibilities and job description for the PHC Clerk San Antonio position at First Home Healthcare, LLC?
The PHC Clerk is responsible for reviewing and keeping the Client's visit log accurate and up to date on a daily basis using the Agency's current program.
When there are missing Primary Home Care (PHC) Attendant call ins and/or call outs the EVV / PHC Clerk will verify the correct time by contacting the client and/or the attendant for verification and will update the Agency program with proper entry and notify the direct field supervisor on all issues that may arise due to discrepancies.
Performance can be measured based on attendants compliance with EVV. Logs must be accurate at all times. The PHC Clerk will comply with required training and adhere to all Agencies’ procedures and policies.

Requirements:
- High School Diploma or GED
- At least one year in a home health setting, preferred
- Strong computer experience
- Bilingual Spanish
- Ability to communicate effectively both in English and Spanish, orally and in writing;
- Strong record maintenance. Experience with gathering and compiling data;
- Basic data entry and/or word processing required
- Highly detail oriented;
- Ability to establish and maintain effective working relationships with all staff, coworkers and general public.
