Lead QA Nurse for Home Health Services

Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our team. The ideal candidate will possess a strong , with a focus on Quality Assurance/Quality Improvement. This role involves chart reviews, OASIS review and training to ensure optimal patient outcomes. The candidate must have a minimum of three years in home health setting (QA experience a plus!)

Responsibilities

• Conduct thorough evaluations of patients care to assess their healthcare needs and assist in the develop of an individualized plan of care.
• Conduct thorough review of patient documentation on referral to ensure client eligibility for home care
• Ensure the accuracy, completeness, and compliance of OASIS assessments and related documentation, collaborating with clinical staff, and identifying areas for improvement in data quality and outcomes
• Coordinate an OBQI program under the direction of the D.O.N.
• Provide education and support to staff along the lines of ensuring quality of care
• Perform chart reviews and evaluations to ensure policy and procedures are maintained
• Review and audit medical charts
• Participate in training and professional development activities to stay current with OASIS practices
• Monitor Outcomes Data Analytics and present areas for improvement

Requirements

• Current Registered Nurse (RN) license in the state of practice.
• Proven experience in OASIS data set and Reviews in home health nursing field,
• Proven knowledge of OASIS documentation and regulations
• Strong understanding of home health Regulatory guidelines
• Knowledge of physiology, behavioral health, medical-surgical practices, and primary care principles.
• Excellent communication skills, both verbal and written, with the ability to collaborate effectively with multidisciplinary teams.
• Ability to work independently while managing multiple tasks efficiently under direction of your D.O.N.
• Able to maintain duties under agency deadlines as directed
