Job Summary
Responsible for following the Person Center Service Plan (PCSP), documenting program
results and communicating with their immediate supervisor about any progress, concerns,
etc. Must be able to work a flexible schedule and fill in hours as needed. This position entails
a high degree of accountability and consequence of error. Ability to stay in close and timely
communication with others is crucial.
Essential Job Functions
In this role, you'll be making a real difference in clients' lives by helping with daily tasks like grocery shopping, running errands, and joining in community activities. Personal care and hygiene are also key parts of your support.
You'll work with several individuals each day, helping them with cooking, cleaning, integrating into the community, and building coping skills. Each day is unique, tailored to meet their specific needs.
Plus, you'll ensure clients get to their appointments, making sure they have access to the services they need to stay healthy and happy.
Requirements: High School Diploma or equivalent. Thorough training will be provided during your employment to excel at this position! No previous experience required! Must have a valid driver's license, a good driving record and show proof of auto insurance.
