Part-time Opportunity: 16-28 hours a week.
Job Summary
Responsible for following the Person Center Service Plan (PCSP), documenting program
results and communicating with their immediate supervisor about any progress, concerns,
etc. Must be able to work a flexible schedule and fill in hours as needed. This position entails
a high degree of accountability and consequence of error. Ability to stay in close and timely
communication with others is crucial.

- Assisting clients with daily needs, such as: Grocery shopping, Running errands, participating in community activities, Personal hygiene and cares, med administration, two person transfers, the use of a hoyer.
- Working with multiple individuals a day assisting with goals like: cooking, cleaning, community integration, and coping skills.
- Take clients to appointments as necessary.
