What are the responsibilities and job description for the Case Manager position at First Step Recovery?
First Step Recovery is seeking a dedicated and compassionate Full-time Case Manager to join our team. Hours are (8am - 4:30pm) Monday - Friday as well as weekends as needed. The Case Manager will provide ongoing professional, technical, and clinical expertise through comprehensive assessment, planning, and treatment, with a focus on coordinating care and addressing individual client needs. This role is crucial in supporting our mission to help clients on their path to recovery.
Key Responsibilities:
- Meet with clients upon admission to discuss the phase process, treatment facility rules, and expectations.
- Serve as the primary point of contact for clients throughout their treatment journey, coordinating their care at First Step Recovery.
- Act as the clinical liaison for client families, providing updates on clinical care, treatment progress, and discharge planning, in accordance with existing Releases of Information (ROI).
- Collaborate with the Clinical Team, client, and client’s family (as applicable) to coordinate and implement effective discharge planning.
- Maintain active files and networks for community referrals and finalize discharge appointments.
- Adhere to a 50% productivity requirement.
- Coordinate transfers to other facilities if necessary, including transfers to higher levels of care.
- Maintain regular and confidential documentation in First Step Recovery’s electronic medical records system.
- Assist with crisis management and clinical intervention as needed. Perform other duties as assigned by the Program Director.
Qualifications:
- High School Diploma or GED required.
- CDCA (Chemical Dependency Counselor Assistant) certificate required.
- Willingness to work with individuals experiencing a wide variety of mental health symptoms and behaviors.
- Ability to work with multicultural populations and an understanding of cultural diversity.
Skills and Competencies:
- Strong interpersonal and conflict resolution skills.
- Ability to exercise sound judgment and maintain confidentiality.
- Excellent organizational, communication, and time management skills.
- Ability to work independently and as part of a team.
- Composure under high-stress situations with the ability to keep personal reactions in check.
- Proficiency in MS Office, including Word, Outlook, and Excel.
