- Discharge Planning & Coordination
- Develop and implement individualized discharge plans for incarcerated individuals with medical and behavioral health needs.
- Collaborate with healthcare providers, mental health professionals, and case managers to ensure proper post-release care.
- Identify and coordinate follow-up medical appointments, prescription medications, and community-based support services.
- Communication & Collaboration
- Work closely with facility healthcare teams, social workers, and external agencies to facilitate a smooth transition.
- Educate individuals on available healthcare resources and self-care strategies post-release.
- Maintain clear and timely documentation of discharge plans and referrals.
- Compliance & Documentation
- Ensure discharge planning meets facility policies, legal requirements, and community standards.
- Maintain accurate records in compliance with HIPAA and correctional healthcare regulations.
- Provide reports and updates to leadership as needed.
- Education: Associate degree in Social Work, Nursing, Psychology, Public Health, or a related field preferred.
- Experience:
- Minimum of 1 year experience in discharge planning, case management, or healthcare coordination.
- Previous experience in corrections, behavioral health, or acute care settings is a plus.
