Position Summary
Hospice Social Worker Job Summary
Responsible for provision of social work services to include explanation of services, crisis intervention, resource referral and emotional support and/or counseling to patients and families as needed.
Ability to work in coordination with other disciplines at part of team to address chronic illness and end-of-life needs.
Complete psychosocial assessments for new patients and explain hospice services.
Participate in the hospice social work call schedule.
Qualifications
Hospice Social Worker Qualifications
Masters Degree in Social Work. NC LCSW, OR LCSW Provisional license preferred. --Must obtain provisional license after hire.
At least 1 year experience in social work required.
Knowledge of end-of-life issues preferred.
Reliable transportation and valid NC Drivers license required.
*Sign-on bonus eligible if fully licensed as a LCSW (not provisionally).
*Does count toward clinical hours for LCSW license
