Position Summary
Hospice Social Worker Job Summary
*Sign-on bonus eligible if fully licensed as a LCSW (not provisionally).
*Does count toward clinical hours for LCSW license
- Responsible for provision of social work services to include explanation of services, crisis intervention, resource referral and emotional support and/or counseling to patients and families as needed.
- Ability to work in coordination with other disciplines at part of team to address chronic illness and end-of-life needs
- Complete psychosocial assessments for new patients and explain hospice services
- Participate in the hospice social work call schedule.
Qualifications
Hospice Social Worker Qualifications
- Masters Degree in Social Work. NC LCSW, OR LCSW Provisional license preferred. --Must obtain provisional license after hire.
- At least 1 year experience in social work required.
- Knowledge of end-of-life issues preferred.
- Reliable transportation and valid NC Drivers license required.
