About the Position
FirstLight Home Care is seeking a Director of Client Care to oversee and manage client care operations in our Campbell, CA office. This pivotal leadership role ensures the highest level of care for our clients while managing our team of Client Care Managers. The ideal candidate is a proactive leader with a strong background in home care, nursing, or case management and a passion for improving client outcomes.
Key Responsibilities
- Conduct initial client screenings and develop comprehensive care plans.
- Oversee and manage Client Care Managers to ensure department efficiency.
- Address and resolve client and caregiver complaints with professionalism and empathy.
- Perform in-home visits and investigations as needed.
- Monitor client schedules and coordinate with the scheduling department.
- Respond to changes in client conditions as reported by caregivers and nurses.
- Collaborate with healthcare providers and community resources for comprehensive care solutions.
- Participate in quality assurance reviews and agency service evaluations.
- Provide leadership and oversight for client satisfaction initiatives.
- Take on periodic on-call responsibilities for after-hours emergencies and client intakes.
