Position Summary
Are you a Registered Nurse licensed in Ohio seeking a highly flexible part-time role? We are looking for an RN Care Manager who can commit to 5–10 hours per week but enjoys the freedom to schedule client visits around personal and professional obligations. Key Responsibilities
Flexible Client Assessments
Conduct initial and ongoing assessments for VA and Medicaid clients—on a schedule that works for you, while meeting client needs.
Recommend care strategies and coordinate with our wider care team.
Oversee Caregiver Competency
Evaluate caregiver skills and ensure they meet state and agency standards.
Provide training, education, and feedback to support caregiver development.
Clinical Documentation & Compliance
Sign off on caregiver and client files, ensuring documentation is accurate and compliant with all regulations.
Maintain HIPAA standards for confidentiality and data security Care Planning & Coordination
Develop and regularly review individualized care plans, collaborating closely with caregivers, family members, and other healthcare providers.
Adjust care plans as client conditions change, ensuring continuity of care.
Medical Resource for Non-Medical Caregivers
Act as an accessible, knowledgeable point of contact for non-medical caregivers seeking clinical guidance.
Participate in case reviews, offering expert insights to improve client outcomes.
Qualifications Required:
Current, active Registered Nurse (RN) license in Ohio.
Minimum of 2 years of nursing experience (home health, hospice, or similar setting preferred).
Preferred:
Experience with VA and/or Medicaid clients.
Prior supervisory or management experience.
Skills & Abilities:
Flexibility and strong time-management skills to design your own schedule.
Excellent communication and interpersonal skills.
Organizational proficiency and attention to detail
Familiarity with state and federal home care regulations.
