Client Care Management
- Oversee all aspects of client care to ensure timely, high-quality services.
- Conduct initial evaluations, develop individualized Service Plans/Care Plans, and implement necessary revisions based on client needs.
- Ensure accurate documentation of client assessments, Service Plans/Care Plans, supervisory visits, and caregiver training.
- Communicate changes in client condition to the care team, physician, and Administrator, ensuring continuity of care.
- Participate in client care conferences and maintain strong relationships with clients and families to provide support and guidance.
- Lead, mentor, and supervise RNs, LPNs, HHAs, and CNAs to deliver consistent, high-quality care.
- Conduct annual performance evaluations, competency assessments, and ensure staff compliance with state regulations and agency policies.
- Facilitate staff meetings, in-service training sessions, and educational workshops to promote ongoing professional development.
- Manage on-call duty rotations, including nights, weekends, and holidays, to ensure 24/7 coverage for clients.
- Foster a positive, collaborative work environment that encourages open communication and teamwork.
- Ensure compliance with state regulations, accreditation standards, and agency policies.
- Participate in quality assurance reviews and implement improvement strategies as needed.
- Monitor service delivery metrics, client satisfaction, and care outcomes to drive continuous improvement.
- Assist with budget planning, resource management, and strategic decision-making.
- Manage scheduling and staff assignments to ensure adequate coverage and timely service delivery.
- Ensure all required reports, documentation, and billing information are accurate and submitted on time.
- Support office operations by providing feedback on care delivery and staff performance.
- Take on additional duties and responsibilities as assigned by the Administrator.
Education & Experience
- Graduate of a state-approved school of nursing with current Registered Nurse (RN) licensure in North Carolina.
- Home Care experience preferred.
- Proficiency in Microsoft Office, Google Suite, and scheduling software.
- Strong leadership, communication, and organizational skills.
- Maintain current RN licensure in North Carolina.
- Possess a valid driver’s license and maintain current auto insurance.
- Exceptional leadership and decision-making abilities.
- Strong critical thinking and problem-solving skills.
- Ability to manage high-pressure situations and multitask effectively.
- Excellent verbal and written communication skills, with the ability to present information clearly to diverse audiences.
- Ability to work collaboratively with team members, clients, and families.
- Ability to read, analyze, and interpret professional journals, technical procedures, and regulations.
- Strong written communication skills for drafting reports and correspondence.
- Comfortable presenting information and answering questions in meetings or client conferences.
