General Summary:

Provide therapy/counseling to ambulatory patients, evaluate patients positive social determinants of health screening, evaluate patients with positive depression screening, work collaboratively with primary care and other behavioral health professionals in the LLC

Essential Functions:

• Assures that social service/discharge planning activities are integrated and coordinated with the hospital/LTC system by effective communication at all levels.
• Provides services to specific patient population, including neonate, pediatrics, adolescent, geriatric and general population.
• Is knowledgeable of and utilizes appropriate community resources.
• Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job-related hazards.
• Maintains standards for licensing in the social work field.
• Follows the hospital exposure control plans/bloodborne and airborne pathogens.
• Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.
• Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmentally to facilitate the department’s/unit’s ability to meet its goals and objectives.
