Job Summary: Supporting patients through transport to and from appointments, handling deliveries and running other errands/completing other assignments as assigned.
Duties/Responsibilities: Work with case managers and clinical staff to establish daily transportation schedules Transport clients to and from approved destinations Pick up and drop off packages as directed Research routes for traffic, construction and weather to maximize efficiency Maintain vehicle readiness/ensure fueled and ready for use and keep mileage and transport records Maintain professional image and communication with clients, guests and vendors and maintain confidentiality/HIPPA rules Patient Assistance: answer questions, assist with mobility or other needs. Contribute to a healthy work environment among staff and patients with a positive attitude and adherence to all policies and procedures. Other related duties as assigned.

Education and Experience:
- High School Diploma or equivalent
- Current Driver's License and Up To Date Insurance
- Clean Driving Record
- Minimum 1 year transportation experience
Physical Requirements:
- On-site and Off-site hours anticipated, at least 6-8/hrs day of sitting and 2-4/hrs day of standing/walking. Daily driving needed. Work days may exceed eight hours.
- Ability to lift up to 50 pounds
