Job Title: Home Health Intake Coordinator
Reports To: Brenda Hubbard
Supervises: N/A
Position Type: Full-Time (Non-Exempt)
The Intake Coordinator is responsible for facilitating new patient’s transition to Fishing Point Healthcare services with the goal of delivering a consistent onboarding experience where patients and referral partners feel Fishing Point Healthcare makes it easy to transition care. The Intake Coordinator works with contracted agencies to prepare patients for home health services and facilitates all aspects of the patient’s transition to home.
PRIMARY DUTIES AND RESPONSIBILITIES:
- Engages with patient, referral source and contracted nursing agency to coordinate discharge and secure home health care.
- Creates, communicates and obtains all required documentation for new referrals.
- Assist Patient Registration Manager with special assignments as needed.
- On-Call Rotation
- Other duties as assigned.
POSITION REQUIREMENTS / QUALIFICATIONS:
- Ability to multi-task and support numerous referrals/priorities at one time.
- Ability to work in a fast past environment.
- Must be detail-oriented and have a high degree of quality focus.
- High degree of customer service skills required.
- Ability to trouble shoot, problem solve and collaborate with cross-functional team members.
