RN Nurse Case Managers

Registered Nurses Needed Fast (Sign on bonus):

Ø WE are Paying a $1,500 - $2,500 signing bonus for this position (to be paid upon successful completion of 90 day probationary period).

Fishkill Home Care has been servicing the residents of Dutchess County since 1982. We offer a team of experienced and dedicated RN Nurse Case Managers, who work together with patients, families, doctors and health care facilities to provide a comprehensive home care program for each client. The primary purpose of this job is to provide skilled nursing care and prescribe treatments in patients homes.

Ø The RN position will service the following designated counties - Dutchess and Putnam Counties;

Responsibilities and Duties:

• Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of home bound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning;
• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care;
• Uses health assessment data to determine nursing diagnosis;
• Initiates the plan of care and makes necessary revisions as patient status and needs change;
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process;
• Initiates appropriate preventive and rehabilitative nursing procedures;
• Provide medication management and education;
• Regularly re-evaluates patient nursing needs;
• Counsels the patient and family in meeting nursing and related needs;
• Provides health care instructions to the patient as appropriate per assessment and plan;
• Manage, evaluate and coordinate care plan implementation with all related disciplines;

Specific Requirements:

• NYS Licensed Registered Nurse (RN);
• One year of experience required;
• Experience with Acute Care preferred;
• Must possess the ability to deal tactfully with personnel, clients, family members, government agencies/personnel and the general public;
• Must have a valid Drivers License;
• Must have patience, enthusiasm and a cheerful disposition;
