JOB TITLE: Family Support Specialist – Non-Exempt
REPORTS TO: Senior Family Support Specialist
DIRECTLY SUPERVISES: Managing Employers/Service Support Provider LOCATION: Client Services Building. Telecommuter eligible as per policy and with supervisor approval. STATUS: Full-Time, Non-Exempt
GENERAL SUMMARY: (General Purpose of the Job)
Perform duties as delegated by Assistant Vice President of Home, Community and Employment Services.
ESSENTIAL FUNCTIONS: (Primary Responsibilities)
• To participate in the ongoing communication, support and training for families and staff working with individuals through Agency with Choice.
• Track ISP Plans, attend ISP (ARU) meetings, attend individual’s team meetings if scheduled for any issues/concerns.
• Work with Service Coordinator (SC) to schedule ISPs for new and existing individuals.
• Advocate for clients to maintain and increase services as their needs change.
• Monitor all HCSIS Alerts for ISP changes/corrections for accuracy in Individualized Service Plans (ISPs) for all individuals.
• Assist with training for families in Agency with Choice for complying with regulations/policies.
• Assists with tracking, maintaining, and collecting all AWC documents encompassing all areas of required paperwork; including compiling/collecting on-boarding packets for managing employers; (acceptable identifying informational documents for CRC background checks) and training manuals.
• Assist with performing managing employer reviews/tracking/filing-uploading.
• Monitor individual’s budgets, services, service hours/units, and utilization ensuring accuracy to remain within budget/overall CAP. (Maintain weekly hours chart)
• Promotes positive reputation of Fitzmaurice Community Services, Inc. programs by acting to resolve any concerns and complaints in a timely manner.
• Follows chain of command and consults with appropriate department executives’ non-routine matters involving individual, fiscal and employee concerns.
• Attends scheduled administrative meetings.
• Schedules phone, virtual or in person on-going meetings/conversation with staff & MEs for concerns/needs and assistance.
• Cooperates with other departments to promote and support agency programs and business strategies.
• Represents the agency at local, state, and national association meetings and functions.
• Ensure all aspects of Incident Management are updated and completed.
• Follow through/oversee training for families in Agency with Choice to comply with regulations and process SSP training time sheets.
CONNECT FUNCTIONS
• Maintain Connect/EVV dashboard, Audit/Approve notes in Connect.
• Prepare weekly EVV SSP payroll statements for payroll processing, track & ensure deadline for submission is met.
• Train/assist MEs with scheduling blocks in Connect when needed, creating base blocks, how to make changes, and how to add scheduling comments.
• Schedule new SSP’s & MEs to attend “What’s in a Note” virtual training. Train with power point. Assist with Connect & EVV training.
• Download all ISP HCSIS Alerts to individual’s files with news feed on ISP read acknowledgement documents; send to all SSPs and MEs for signatures.
• Add new (ARU) Individual’s plans (New Outcomes) into Connect with accuracy for correct start/end outcome dates and review from Senior FSS
• Assist with notifying Director of Technology of all staff changes (additions, moves, terminations) prior to the change or immediately after. Terminations should be made known prior or immediately after termination meeting so that access can be stopped.
• Assist with notifying Fiscal Assistant of any Individual movements.
• Write quarterlies within 15 business days of end of period, submit to Senior FSS for review. (Upload to Connect)
• Maintain AWC client photos/consents in Connect (Tracking).
• Assist with uploading bi-weekly (budget) temp statements to Connect & e-mail ME for viewing.
QUALIFICATION STANDARDS:
• 18 years or older
• Computer knowledge; Microsoft office business, Excel, and Publisher
• Valid and Insurable U.S. Driver’s License
• Physical ability to perform all duties associated with position including lifting and carrying techniques up to 25lbs.
• Properly perform the techniques taught in CPR/AED/First Aid.
