GENERAL SUMMARY:
Working within a variety of services to offer support to individuals to help them reach their goals. Provides support to ensure individuals achieve goals for the services of Employment/IHCS/Companion and Supported Living. Engage individuals and establish trusting, collaborative relationships in community settings. Provide support to ensure individuals progress while providing support that is consistent with their goals.
ESSENTIAL FUNCTIONS:
- Arrive on time at the specified site.
- Assist the individual in performing the work responsibilities in such a way that fosters independence.
- Create and maintain positive relationships with the individual, supervisors, and co-workers in the workplace.
- Continually assess the vocational, social, communication and independent skills required by the specific job setting.
- Perform task analysis of each task as needed and provide guidance to participant/client.
- Identify job site modifications as needed. Recommendations are to be discussed with the AVP of the Department.
- Comply with the policies and procedures of the job sites.
- Report to the AVP any of the individual’s needs, problems, or concerns.
- Complete service notes for each shift worked with the individual in Connect.
- Encourage, guide and train individuals in developing skills in accordance with their ISP.
- Provide a safe, clean, comfortable, and therapeutic environment during activities in the community.
- Experience and/or education working with individuals with disabilities in the employment services or equivalent service field.
- Valid and Insurable U.S. Driver’s License
- Great attention to detail and a high level of professionalism.
- Ability to interact and interface with employees and employers at all levels of the organization.
- Demonstrated ability to work well under pressure, handle interruption and adjust priorities.
- Strong independent work skills.
- Computer knowledge; Microsoft office business
- Properly perform the techniques taught in CPR/ First Aid.
- Properly perform lifting and carrying techniques. (Lift 25lbs)
- Candidate will have the ability to independently research community activities appropriate for the individuals they are working with.
- Required at times to use personal vehicle for transport to community activities.
- Have a device to access and use EVV
