Position Title: Education and Outreach Specialist (Meridian, MS)
Reports To: Director of Outreach (MS)
Position Purpose:
The Education and Outreach Specialist position is responsible for developing and implementing outreach activities that reach priority populations at Mississippi State University-Meridian Campus, Meridian Community College, East Mississippi Community College-Naval Air Station Meridian, and in the Meridian, MS community at large. Health education topics center on HIV/AIDS, STIs, PrEP, stigma, health equity and other public health initiatives. Education and Outreach Specialist strives to make individuals and communities aware of their HIV status, conduct prevention services, linkage, and other programs available at Five Horizons Health Services. Critical functions of this position are community wide HIV/STI testing, networking, public speaking, event coordinating, and providing education and prevention programs to the community at-large about the health impact of HIV. Education and Outreach Specialist also empower individuals to make healthy life choices. Essential Functions: Through a variety of means, actively engage students, groups, and faculty/staff at Mississippi State University-Meridian Campus, Meridian Community College, East Mississippi Community College-Naval Air Station Meridian, and Meridian, MS community at large consistently increasing the agency’s on-campus scope of reach. Provide integrated STI/HIV testing for Mississippi State University-Meridian Campus, Meridian Community College, East Mississippi Community College-Naval Air Station Meridian, and Meridian, MS community at large and surrounding counties that includes Meridian, MS service area. Identity people living with HIV in the Meridian, MS community and connect them with early intervention services: HIV testing, counseling, and referrals to care. Via a ‘warm handoff’, link relevant students and community members to HIV medical care/case management at Five Horizons; Link all HIV negative client to PrEP services via the Five Horizons’ PrEP Navigator Make all efforts to ensure retention in care for referred HIV and PrEP patients Provide referrals to holistic care for Mississippi State University, University of Mississippi for Women, East Mississippi Community College, and MS service areas. (i.e, mental health, primary care, transportation) Provide behavioral health assessments as appropriate (i.e., PHQ-9, GAD-7, ASSIST, AUDIT-C) Ensure completion of all relevant documentation and data collection (i.e., PrEP referrals, post test education) Collaborate extensively with Chief Outreach Officer and Director of Public Relations to ensure culturally-relevant marketing strategies are utilized, including effective use of website and social media channels in accordance with Mississippi State University-Meridian Campus, Meridian Community College, East Mississippi Community College-Naval Air Station Meridian marketing protocols. Assist with FHHS-MS location wide Outreach and Education services as requested by Director of Outreach. Maintain files and data for reporting purposes and contribute to programmatic reporting requirements. Perform other duties as assigned.
