Reports To: Manager of Supportive Services
Position Purposes: The Intake and Retention Specialist is a motivated and compassionate individual responsible for the daily operation and execution of the Intake Process as well as coordinating with other departments to ensure clients are successfully engaged. The Intake and Retention Specialist works as part of an interdisciplinary health team to coordinate linkage to care, identify and minimize barriers to care, and facilitate the ongoing delivery of social services to clients of Five Horizons Health Services (FHHS).
Essential Functions:
· Administer non-invasive HIV tests and complete required paperwork
· Provide pre- and post-test HIV education and counseling
· Engage in routine communication with new clients to facilitate an effective transition into care
· Identify and develop a plan to re-engage clients who are lost to care
· Complete walk-in and / or telephone intakes with all new clients as the initial step in the engagement process
· Occasionally meet with clients outside of the office (client’s home, hospital, etc.) to complete intakes
· Collaborate with medical providers, social workers, and other professionals to identify and address medical needs related to access / retention to care and treatment
· Demonstrate a skilled knowledge of engagement, assessment, case planning, intervention, and monitoring techniques to facilitate retention to care for targeted clients
· Monitor clients who are returning to care to facilitate engagement and ensure needs are identified, with an emphasis on high-risk clients
· Maintain client records including timely documentation in the Electronic Medical Record (EMR) system or other database in accordance with departmental policies and procedures
· Complete applications and determine eligibility for services provided
· Maintain compliance with HIPAA, state guidelines, FHHS policy, and uphold confidentiality of clients
· Perform any other departmental or agency-related duties as assigned
Minimum Qualifications:
· High School diploma / GED with two (2) years’ experience in HIV-related field or
· Bachelor’s degree in social science or health-related field with previous experience in HIV care or
· Bachelor’s degree (any field) with one (1) year of direct HIV case management experience
· Excellent interpersonal skills

This position requires an individual who is self-motivated and able to independently complete assigned responsibilities. The individual must also be able to accept proper supervision and work cooperatively with clients, peers, and other professionals.

Pay: From $42,000.00 per year
