Five Keys Overview

Five Keys was founded in 2003 by the San Francisco Sheriffs Department as the first accredited charter high school in the nation to provide diploma programs for adults in county jails. Today we are a much different social justice non-profit agency that has expanded its charter schools into 9 counties, in 24 jail locations and over 80 community learning centers. In addition to our schools, now we also run multiple homeless shelters, employment programs, programs for the unsheltered, reentry programs and housing for women suffering from immense injustice.

JOB SUMMARY

The TAY In Custody Case Manager/Facilitator reports to the Director of Re-Entry & In Custody Programs, and is accountable for supporting 18 to 24-year-old clients and students who are enrolled in case management and resilience programming in the San Francisco County Jail.  This includes conducting groups, developing case management plans and connecting clients with community and government resources.

You are also comfortable working with employees from every walk of life, including previously incarcerated individuals.  This position requires a high level of empathy, patience, and compassion and the ability to meet the needs of our students and clients using a trauma-informed approach, while upholding the mission Five Keys.

 

JOB DUTIES AND RESPONSIBILITIES

Case Management Duties

  • Conduct groups, develop case management plans and connect clients with community and government resources
  • Propose and implement clinical case management and case conferencing strategies
  • Support other Five Keys TAY In Custody Case Managers/Facilitator in the implementation of clinical case management best practices
  • Develop systems and processes that could lead to a more creative and rich learning environment
  • Develop systems that would increase utilization in Five Keys services for TAY students and clients
  • Support TAY students and clients in the pod to ensure they are utilizing Five Keys services
  • Create a protocol outlining case conferences
  • Develop and maintain records on participating clients, documents case records and reviews reports
  • Compile and analyzes data and prepare summary reports

Facilitation Duties

  • Provide effective programming and facilitate CBT and evidence based curriculum
  • Develop and/or use curricula that would be beneficial to clients
  • Create and maintain services designed to reduce recidivism and promote well-being for our clients
  • Collaborate with managers in the delivery of programs to meet the specific needs of incarcerated men and women, including support groups, courses, academic tutoring, parenting classes and other therapeutic and recreational activities
  • Liaise with SFSD, Community program partners and APD to ensure clients have seamless access to a continuum of services
  • When requested, attend public forums and collaborative meetings to promote access to a continuum of services for clients exiting incarceration
  • Implement outreach procedures
  • Track and document outcomes
  • Facilitate effective communication of information to various departments
  • Communicate with APD TAY Pre Release Coordinator, DPOs and Pod Deputy regarding facilitators
  • Fill in for facilitators on an emergency basis and troubleshoot procedure to prevent future disruption
  • Troubleshoot with facilitators and Pod Manager when there is a programmatic or logistical issue
  • Ensure that attendance sheets are collected, and entered into the DCYF and Five Keys databases
  • Conduct and oversee a peer-led weekly community meeting with clients

General Duties

  • Provide a welcoming and supportive environment for all clients and colleagues
  • Coordinate with CBOs and government organization to establish connections to and on-site provision of client resources such as housing, public benefits, employment services, substance abuse treatment, etc.
  • Support all administrative tasks
  • Provide information requested by Five Keys Administration and DCYF
  • Maintain accurate records and log all referrals made in designated databases
  • Act as the immediate liaison between Case Managers, Partners, Five Keys, DCYF, and Sheriff's Department
  • Work with Sheriff's Department, the Reentry Pod DPOs, and post case managers to arrange for release pick-ups/ warm hand offs
  • Document any programmatic protocol violation accompanied by an incident form
  • Abide by Five Keys and Sheriffs Department policies and procedures
  • Provide quality assurance in regards to programming and overall program operations as they relate to all TAY services
  • Other duties as assigned

 

KNOWLEDGE AND SKILLS

  • Proven skills in facilitating groups and providing case management
  • Demonstrated experience working on issues of complex trauma, parent-child relationships, attachment, service integration, and holistic interventions that are culturally responsive
  • Knowledge of and practice in applicable recovery models including harm reduction
  • Ability to demonstrate comfort, maturity, and professionalism when interacting with diverse community partners, custody staff, students, and colleagues
  • Proficiency in Zoom, Microsoft Office, Google Workspace, and databases
  • Willingness and ability to grow professionally, be accountable, and to expand skills and adapt to evolving school environments is required
  • Ability to maintain strong and appropriate boundaries with personal identifying information when interacting with students or community or program partners, and while at any school location or community or program partner site
  • Exceptional written communication skills; ability to express complex ideas with clarity and precision in written documents
  • Ability to handle multiple deadlines and tasks simultaneously
  • Eagerness to learn new things and able to adapt to constantly changing circumstances
  • Ability to interact with those we serve through a trauma-informed approach and maintain strict confidentiality when handling sensitive information
  • Advanced problem-solving skills and the ability to tactfully address and resolve questions/concerns
  • Knowledge of the challenges and need of Five Keys students and the ability to commit to the organization’s mission, vision and
  • Exceptional written and verbal communication skills and the ability to express complex ideas with clarity and precision in written documents
  • Able to adapt to constantly changing circumstance and demonstrate willingness to learn new things

 

EDUCATION AND WORK EXPERIENCE

  • Must pass Department of Justice fingerprint screening
  • 2 years case management, counseling, and assessment experience
  • 2 years experience working with reentry or in-custody populations experience preferred OR
  • Equivalent combination of education and experience
  • Experience working on issues of complex trauma, parent-child relationships, attachment, service integration, and holistic interventions that are culturally responsive

 

PHYSICAL REQUIREMENTS

  • Must wear provided PPE (Personal Protective Equipment) and adhere to all safety guidelines and requirements, when needed
  • Lifting items that weigh up to 25 lbs.; reaching, bending, carrying, and stooping within the daily routine of tasks and activities
  • Frequent talking and hearing conversations
  • Travel to multiple sites required

 

ADDITIONAL REQUIREMENTS

  • This position is contingent upon:

  • Satisfactory results of a Live Scan (criminal background investigation)
  • Successfully obtaining jail clearance for positions working in county jails or probation locations
  • Submitting proof of clear TB results. Results dated within 60 days prior to hire date are acceptable

 

WORK ENVIRONMENT

  • Moderate to high noise level
  • Work environment varies from site to site
  • Some personal safety risk working in underserved communities
  • Must be willing and able to work in a correctional facility, travel throughout the area to community sites, and travel state-wide to various locations or meetings as needed
