Position SummaryProvide senior population of Flagler County with a single-entry point into the community care and social services program system.
Essential Functions
- Assists the Senior Services Program Manager in the administration of program objectives.
- Successfully completes all state required components of the DOEA 701B Comprehensive Assessment, 701A Condensed Assessment, 701C Congregate Meal Assessment, and 701S Screening Assessment as updated July 16, 2013. (This will include the completion of all required assessments, updates, etc., for incorporation into individual files by client and program.)
- Coordinates services according to DOEA Programs & Services Manual, meeting program compliance and County standards.
- Provides case management in accordance with the Department of Elder Affairs Program & Services Manual. The case manager will provide comprehensive file narratives in each individual file to meet established standards.
- Case manager shall make required home visits in all points of Flagler County, i.e., city and rural boundaries.
- Establishes eligibility and a plan of care for Title III B in-home services, i.e., homemaking and personal care, and serves as liaison with vendors, staff, and clientele. Maintains on-going communication with the client to address matters of quality assurance.
- Establishes eligibility for the Title III-C-1 Congregate Meals program and processes all appropriate paperwork for tracking units of service. Coordinates with Site Leader for assistance in the required file reviews and completion of all applicable applications and forms related to Title III-C-1 Congregate Meals Program
- Establishes eligibility for Title III-C-2 Home Delivered Meal clients and processes all appropriate paperwork for tracking units of service.
- Establishes eligibility for the Emergency Home Energy Assistance for the Elderly Program (EHEAP). Completion of all file components to meet compliance standards.
- File maintenance for all Title III programs, i.e., III-B, III-C1, and III-C2 must be completed; files must meet all phases of monitoring compliance to include: legible and comprehensive file narratives/notes; required forms and applicable file documentation, etc.
- Maintains client confidentiality, including handling client files according to HIPAA requirements and shredding of sensitive client documents as prescribed by program and organizational policy.
- Seeks out new clients via community/ neighborhood outreach to encourage involvement in program activities.
- Maintains accurate, current data/records on service provision and submit forms as required. to include all necessary requirements/components for the Florida Department of Elder Affairs’ Client Information Registration and Tracking System (e CIRTS).
- Promotes project income for Title III services and various department projects.
- Actively advocates for the client.
- Informs supervisor of all areas of concern and incidents not in accordance with normal daily routine and job function in a timely manner.
- Assists in the preparation of the Title III grant application.
- Participates in program monitoring by Area Agency on Aging, meeting requirements of the Department of Elder Affairs.
- Works with Supervisor in maintaining current procedure/policy manual.
- Attends staff meetings, workshops, seminars, conference, and trainings as required.
- Performs related work as required.
- In the event of an emergency, all employees are considered essential personnel and shall be required to perform alternate duties.
- Bachelor’s degree in Human Services, Social Work, Gerontology or related field, or an equivalent combination of education, training, and experience year for year.
- Minimum of two-years work experience in a social services setting.
- Must possess a valid State of Florida Driver’s License.
