Title: Infusion Nurse

FLSA Status: Non-Exempt

Reports To: Director of Nursing

Position Summary

The Infusion Nurse is responsible for providing in-home and in-suite nursing services to assigned patients in accordance with the patient’s plan of care. The Infusion Nurse will be responsible for safely preparing and administering parenteral medication and providing education related to prescribed therapy regimens. The Infusion Nurse will collaborate with the internal and external prescribers to ensure safe, effective patient care with optimal outcomes.

Job Duties

  • Prepare and administer medications in the home and/or infusion suite settings in accordance with the manufacturer’s prescribing information, prescriber’s orders, professional scope of practice, and laws and regulations for both adult and pediatric patients
  • Evaluate the home for safety and appropriateness for home infusion therapy
  • Complete clinical documentation of all patient care activities performed accurately and in a timely manner within the electronic medical record and/or on paper, in compliance with internal and external regulatory requirements and in support of organizational billing processes
  • Observe and monitor patient response/tolerance during all patient care activities
  • Perform intravenous, intramuscular, and subcutaneous medication preparation and administration activities in compliance with the manufacturer’s prescribing information, prescriber’s orders, and industry best practices
  • Ensure all pre-infusion requirements have been met prior to delivery of care, including clinical documentation and required legal and regulatory documents
  • Perform initial patient evaluations and assessments and develop and communicate the written plan of care/treatment plan
  • Ensure all patient care activities are performed within scope of practice and with a goal of optimal patient outcomes and safety
  • Provide initial and ongoing patient/caregiver education per organizational policy and accreditation standards
  • Counsel the patient and caregiver in meeting therapy-related goals
  • Regularly reassess the patient’s condition and nursing needs and update the plan of care/treatment plan as required
  • Assist the pharmacy in the entry, review, and validation of prescriber orders
  • Evaluate and re-evaluate the patient’s need for continued services
  • Perform lab specimen collection activities and transport to a local lab for processing as needed
  • Assist in the ongoing review of lab results, contraindications, and abnormal findings, and report critical values to prescriber as appropriate
  • Ensure ongoing care coordination with the prescribing provider regarding changes in patient condition and any deviations from the plan of care/treatment plan
  • Perform all activities in compliance with local, state, and federal regulations and organizational policies and procedures
  • Ensure complete understanding of accreditation standard requirements and implement best practices throughout all daily activities
  • Recognize and respond to side effects and adverse reactions in a timely and effective manner per organizational policy and prescriber orders
  • Ensure coordination of care is maintained with all applicable care team members within the organization
  • Perform cleaning and disinfection of patient care equipment per local, state, and federal regulations, accreditation standards, and organizational policies and procedures
  • Demonstrate competency in providing patient care in accordance with professional standards and applicable scope of practice
  • Participate in surveys conducted by authorized inspection agencies
  • Report any misconduct, suspicious, or unethical activities to the Compliance Officer
  • Attend continuing education programs and staff meetings as appropriate per role
  • Represent the organization in a positive and professional manner
  • Perform other duties as assigned by supervisor

Supervisory Responsibility

This role does not include supervisory responsibilities.

Work Environment

This role operates in various clinical settings, including the infusion suite and patient home. This role routinely uses standard office equipment, computers, phones, emergency equipment, infusion equipment and supplies, and patient monitoring equipment.

Position Type and Expected Hours of Work

This is a PRN position with typical working hours of 9:00 a.m. to 5:00 p.m. Monday through Sunday. Hours and work shifts may change, including the addition of evening hours in accordance with patient needs.

Required Minimum Job Qualifications, Experience, Education, and Training

Education: Associate Degree in Nursing (ADN) and an active applicable state nursing license

Experience: 2 years direct patient care in specialty infusion, home infusion, dialysis, or related field; strong IV access skills

Certification(s): Active BLS/CPR required upon hire; ONS Fundamentals of Chemotherapy and Immunotherapy Administration Certification, required within ninety (90) days of hire, if full-time

Computer Skills: Previous experience working with electronic medical record systems

Additional Requirements: Unrestricted motor vehicle license; active automobile insurance

Preferred Education and Experience

Education: Bachelor of Science in Nursing (BSN)

Experience: 5 years in infusion therapy, dialysis, vascular access, or other directly related nursing specialty with experience in the management of patients on specialty medications (i.e., IVIg, Monoclonal Antibodies)

Certification(s): Nursing certification in related field; Certified Registered Nurse Infusion (CRNI), Immunoglobulin Certified Nurse (IgCN)

Computer Skills: Proficient skill level in Microsoft Office, Excel, and Word

Additional Eligibility Qualifications

Must be able to accomplish any and all duties with little to no assistance.
