What are the responsibilities and job description for the Case Manager - Hire LAX position at Flintridge Center?
As the Reintegration Case Manager, you will be a member of the Reintegration Team. You will assist community members (Constituents) in difficult situations with advice, services, resources, referrals and advocacy. Your goal will be to help Constituents achieve sustainable, healthy lives. To achieve this goal, your work will involve assessing, planning, implementing, monitoring and evaluating.
Note: This position will be located full-time at the Los Angeles County Probation Department Foothill Area Office in Pasadena, CA.
PRIMARY RESPONSIBILITIES:
Provide systems navigation and case management services to adult probationers, including emerging adults ages 18-25 at the LA County Probation Department Foothill Area Office in Pasadena, CA.
Assist Constituents with the following types of services:
Provide case management services, based on agreed upon standards, procedures and policies, including needs assessments, linkages to services, and maintaining eligibility documentation.
Assist Constituents in determining what kind of help they need;
Develop care and service plans for constituents
Provide systems navigation services for Constituents, always seeking services and resources that are focused on safe, timely, effective, efficient, equitable and Constituent-centered care and assistance;
Work with community partners to whom we refer Constituents to track their progress with treatment plans;
Maintain daily sign-in logs and documents outlining referrals and services
Provide monthly reports to LA County identifying services provided
Maintain an honest and ethical perspective while sharing information about services, i.e., be clear about the fact that systems navigation, case management or completing the ARP course do NOT guarantee a result. Rather, that it will require hard work and persistence in order to successfully reintegrate back into the community. Flintridge is here to support successful reintegration.

Educational
You must meet the following minimum requirements to be considered for the Reintegration Case Manager position:
Must have a certificate in case management or equivalent education/experience.
Personal and Professional
You will be working with community members who are sometimes in dire situations or are going through difficult, painful transitions. First and foremost, you must be compassionate to their situations and work with them in a positive, patient, thoughtful, caring way that will help them make good forward strides. At Flintridge, we approach our work from a trauma informed perspective.
You will also need a tolerance for stress, ambiguity and change and the ability to maintain composure under pressure.
Must have experience working with previously incarcerated community members.
Must be self-motivated and have the ability to work independently and to act responsibly.
Must have experience in working in a culturally and economically diverse community.
Must be comfortable working at LA County Probation Department’s Foothill Area Office.

Pay: $27.00 - $35.00 per hour
