Completed prior to start and updated every 3 years. HIPAA, Joint Commission -NPSG, OSHA Topics. Completed within a year of start updated yearly. Minimum 80% score. Only 2 attempts allowed. Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted. Required for all travelers. Preferred to be completed prior to start, can be completed onsite. PAPR will not be completed on site. WI and MI require 3M 1860 Regular, 3M 1860 Small or 3M 1870. Updated annually. Documentation of vaccination series completion OR documentation of declination to vaccine to be signed and dated by traveler. Flu season is October 1, 2024 through March 31, 2025. Exemptions / Declinations are accepted, but candidate will be required to wear a mask at all times. 1st PPD within 1 year, 2nd PPD within 30 days of start. And / or Negative IGRA within 90 days of start date lab printout prior to start. TB Questionnaire must be updated annually. If positive TB record; Requires Proof of Chest X-Ray within last 1 year to be accompanied by TB questionnaire. Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider. If negative, provide proof of two doses of Varicella vaccine, four weeks apart. No declinations accepted. Valid 10 years from the administered date. No declinations or attestations accepted.
