Job Summary
The Nurse/Health Educator is responsible for health education and program participant access to medical care, playing a critical role in ensuring participants receive the education and resources to be active and informed decision makers in their and their children’s health care. Under the supervision of the Director of Client Care, the Nurse Educator serves as a resource to agency staff, maintains and updates policies and procedures in coordination with agency leadership, and provides in-service training to clients and staff, such as medication education and administration, prenatal care, and reproductive health.
Requirements for the Position:
- Women’s Health Expertise: Registered Nurse or Physician Assistant actively licensed in South Carolina with at least four years of nursing experience, at least two years of which are in Obstetrics, Maternal and Child Health and/or Women’s Health.
- Active Listening and Communication Skills: Ability to listen, ask the right questions and recognize non-verbal cues. Excellent verbal and written communication skills to provide information to individuals with varying cognitive abilities.

- CRM/Case Documentation Proficiency: Strong knowledge and practical use of electronic case management systems for individualized care planning, case documentation, and outcomes tracking.

- Transportation: Ability to drive a vehicle, possess reliable transportation, and a valid SC driver's license.
- Physical Demands: Ability to operate standard office equipment and keyboards. Frequently stand for long periods of time; use hands to handle or feel objects. Occasionally sit for long periods of time, walk, climb stairs, kneel, crouch, and reach or grasp with hands and arms. Occasionally lift and/or move up to 20 pounds. Close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading. Specific Responsibilities:
Service Delivery (75%):
- Preview medical records of potential participants to identify needs and appropriateness of agency services.
- Conduct initial health screening for all new program participants and establish healthcare and pharmacy services.
- Collaborate with participants’ Success Coaches to support participant learning and overall well-being, including:
- development and monitoring of individualized health goals of participant care plans
- attend weekly multidisciplinary team meetings to provide information and resources to compliment the health goals of participant care plans.
- Supervise medication administration and documentation, including managing online Medication Administration Record system, training staff, and communicating medication changes with staff and pharmacy.
- Conduct weekly office hours at multiple locations throughout the Tricounty area to empower participants to access health care, ensure medication and care plan compliance, and provide resources to promote overall well-being.
- Ensure all participants establish primary, obstetrical, and pediatric care providers.
- Ensure participants receive medical, vision, and dental services as recommended by the American Association of Pediatrics and as required by their primary care providers.
- Identify evidence-based health education curricula and conduct or supervise weekly group health education.
Administrative (25%):
- Document all services provided, verify participant health insurance, and oversee medical billing, where applicable.
- Track health and well-being statistics and participate in program quality improvement activities.
- Ensure all residential program homes maintain adequate medical supplies and dispose of medications in accordance with agency policy.
- Conduct monthly audits of all participant health data and submit reports as required by regulatory agencies.
- Coordinate with agency leadership to keep participant health policies and procedures up-to-date..
- Complete a minimum of 16 hours of job-related continuing education annually.
- Represent Florence Crittenton in community partnerships and activities, e.g. working groups and task forces, community events, speaking engagements, presentations, etc. as assigned by the Sr. Director of Programs.
