SUMMARY: Responsible for designing and implementing individualized service plans for each participant to achieve self-sufficiency, vocational success, and positive family development, aligned with Crittentons mission of empowering women and children through education, health, residential care, and supportive services.
REPORTS TO: Social Work Supervisor
STATUS: Full-time, nonexempt
RATE OF PAY: $22.83 per hour

Using a strengths-based and trauma-informed approach, the Aftercare and Vocational Support Specialist acknowledges ongoing care and connection in clients' transitions. Provides comprehensive services and support to former clients from all residential programs (Maternity Hall, Sarahs House, and Legacy Hall) who have transitioned out of care. The Specialist focuses on empowering clients to achieve self-sufficiency by addressing educational, vocational, parenting, and personal development goals. This position includes an emphasis on job readiness and vocational programming, community referrals, and helping clients establish and maintain stability in all aspects of their lives. The ideal candidate will have experience working with at-risk youth, populations who have experienced trauma, and in residential group living environments, along with excellent customer service and communication skills.

Specific Duties and Responsibilities
General Program Duties:
- Conduct weekly/bi-weekly home visits or virtual check-ins with program participants to support educational, vocational, and health goals.

Develop and implement individualized service plans within 30 days of initial engagement, addressing:
- Educational advancement
- Vocational readiness
- Housing stability
- Medical and health education
- Family planning and parenting skills
- Social and community connections
- Ages & Stages
- Collaborate with current staff to engage residents in the program before transitioning out, building trust and familiarity with program offerings.
- Use strengths-based and trauma-informed approaches to motivate participants to make positive lifestyle choices and work toward their goals.
- Identify and help participants develop formal and informal community supports.
- Make community referrals for participants to address needs such as food, clothing, childcare, and transportation, ensuring timely access to resources.
- Maintain a detailed, up-to-date list of community referrals and resources to assist clients effectively.
- Responsible for the collection and data entry of APGAR scores and other birth data from all Crittenton deliveries.
- Document program activities, participant progress, and outcomes in Extended Reach.
- Abide by the NASW Code of Ethics.
Vocational Program Duties:
- Conduct individual assessments to determine clients job readiness and vocational goals; counsel clients to address barriers to success.
- Coordinate with community partners to facilitate individual and group sessions on job preparation, rsum writing, interviewing, money management, and independent living skills.
- Work with clients to develop measurable vocational and educational goals and monitor their progress.
- Build relationships with community organizations, businesses, and training providers to create job placement and training opportunities for clients.
- Coordinate in-house job placement programs to provide hands-on vocational experience.
- Transport clients to and from approved destinations related to educational or vocational goals.
- Organize workshops and guest speakers on topics such as work readiness, career planning, and financial literacy.
- Recognize client achievements with regular agency-wide celebrations.
- Leverage community resources to enhance vocational programming and opportunities.
- Ability to engage and motivate clients from diverse backgrounds using a culturally competent approach.
- Strong understanding of trauma-informed care and its application in human services.
- Demonstrated ability to build partnerships with community resources and employers.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work flexible hours, including some evenings and weekends.
- Must have clean driving record, valid driver license, and the ability to travel locally.

Education and Experience
- Associates degree in Human Services, Social Work, or a related field AND one year of experience in human services, residential care, vocational training, or related areas, OR
- High school diploma or equivalent AND at least two years of experience in human services, residential care, vocational training, or related areas.
