Duties
- Supervise and coordinate support services for individuals with developmental disabilities.
- Develop and implement individualized support plans tailored to each client’s needs.
- Manage relationships with consumers, families, service providers, and community resources to ensure comprehensive care.
- Oversee program development initiatives to enhance service delivery and client satisfaction.
- Maintain accurate documentation of services provided, including progress notes and budget management.
- Conduct regular assessments of client needs and adjust support plans as necessary.
- Facilitate communication between clients and their support networks to promote advocacy and empowerment.
- Ensure compliance with all regulatory requirements related to service provision.
Requirements
- Bachelor’s degree in social work, psychology, or a related field; APD Medicaid Authorization number preferred
- Experience working with individuals with developmental disabilities is preferred.
- Proficiency in administrative tasks and program management.
- Knowledge of behavioral health practices and principles is a plus.
- Ability to manage multiple priorities while maintaining attention to detail.
- Strong communication skills, both verbal and written.
