Job Title: LPN Care Manager
Reports to: Manager, HealthSync
Job Summary: Works collaboratively with physicians to manage groups of patients through the continuum of care settings in an effort to improve patient outcomes and promote evidence-based medicine.
**THIS IS A REMOTE POSITION - CANDIDATES MUST RESIDE IN FL, PREFERABLY THE IMMEDIATE TAMPA BAY REGION**
Minimum of 2 years experience in a similar role preferred. Hospital follow-up experience and ability to communicate with patient telephonically required. Bilingual a plus!

Essential Functions of the Position:
- Evaluates and identifies patient needs
- Interfaces with primary care physicians, specialists and various disciplines on the development of care management plans
- Implements care/treatment plans by coordinating access to health services across multiple providers/disciplines/settings
- Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination and discharge planning
- Promotes self-care management through health promotion and education
- Monitors care delivery and outcomes
- Collaborates with physicians to ensure evidence-based guidelines are implemented
- Identifies cost-effective measures and makes recommendations for alternate utilization of resources
- Participates in the formulation, implementation and monitoring of action strategies and outcomes of care and patient/caregiver experience
- Acts as a clinical liaison with outside agencies
- Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued care/treatment
- Performs data collection and analysis for selected preventive and quality metrics
- Assists in using results to improve care and service
- Promotes patient engagement and shared decision-making
- Interacts with patients and family regarding individualized patient/family education plans
- Communicates clinical knowledge/evidence-based medicine to patients in a manner that is understandable
- Performs other incidental and related duties as required and assigned.

- Ability to move frequently throughout the facility and to sit for long periods of time
- High level verbal and written communication skills
- Above average ability to manage multiple tasks and projects simultaneously
- LPN with valid Florida license
- Minimum of two years clinical experience
- Minimum of two years experience in case management, utilization management or discharge planning
- Excellent communication and problem solving skills
- Strong customer service orientation
- Computer proficiency in Word, Excel and other applications
- Bilingual a plus
