Nurse Practitioner

Job Overview
We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care, with the ability to work independently and collaboratively within a multidisciplinary team. This role involves providing high-quality healthcare services, including assessment, diagnosis, and treatment of patients in an outpatient setting. The Nurse Practitioner will also engage in health coaching.

Duties

• Conduct comprehensive health assessments and physical examinations of patients.
• Develop and implement individualized care plans based on patient needs and medical history.
• Administer medications and perform phlebotomy as required.
• Maintain accurate medical records using Practice Fusion
• Provide health coaching to patients, promoting wellness and preventive care strategies.
• Collaborate with physicians and other healthcare professionals to ensure coordinated care.
• Educate patients about their conditions, treatment options, and preventive measures.
• Demonstrate knowledge of physiology to inform patient assessments and interventions.

Requirements

• Current Nurse Practitioner license in the state of practice.
• Experience in an outpatient setting preferred
• Familiarity with electronic medical records systems, particularly Practice Fusion
• Strong understanding of clinical research methodologies is desirable.
• Excellent communication skills with the ability to provide compassionate patient care.
• Ability to work independently while also being an effective team player.
• Knowledge of health coaching principles to support patient engagement in their health journey.
