- Advocate for residents’ rights and well-being, providing support for psychosocial issues, including discharge planning, adjustment to facility life, and mental health concerns.
- Provide emotional and psychosocial support to residents and their families, facilitating communication and conflict resolution as necessary.
- Ensure accurate and timely documentation, maintaining confidentiality and compliance with healthcare regulations (HIPAA).
