Position Summary:

The CSED Wraparound Facilitator at Flourish Health is responsible for coordinating services, assessing needs, and developing individualized care plans that align with the CSED initiative in West Virginia. This role involves working closely with families, the Flourish care team, service providers, health plan case managers, and community resources to deliver comprehensive, holistic case management. The facilitator will also coordinate and lead in-home CSED care meetings, ensuring seamless collaboration and support for youth and their families.

Key Responsibilities:

Wraparound Team Coordination:

• Plan and facilitate Wraparound Team meetings involving youth, families, and service providers.
• Develop and implement individualized Wraparound care plans based on assessments and family input.
• Identify and engage resources available to youth and families.

Family and Community Engagement:

• Build rapport and maintain ongoing communication with families and community service providers.
• Conduct home visits as needed to provide direct support to youth and families.
• Provide education and guidance to families on navigating mental health and social services.
• Identify service gaps and collaborate with stakeholders to address barriers to care.

Assessment and Documentation:

• Obtain and maintain proficiency certification to administer CANS (Child and Adolescent Needs and Strengths) assessments.
• Complete required assessments and maintain accurate documentation in compliance with regulatory standards.
• Monitor and track youth and family progress, adapting care plans as needed.
• Produce written reports and maintain client records according to organizational and CSED guidelines.

Qualifications:

Required:

• Bachelor’s degree in human services, social work, psychology, or a related field.
• Experience working with children and families in mental health or social services.
• Certification to perform the CANS assessment or willingness to obtain certification.
• Completion of training in Wraparound Facilitation and National Wraparound Initiative (NWI) principles.
• Strong knowledge of community resources and inter-agency collaboration.
• Ability to work independently and as part of a multidisciplinary team.
• Excellent communication, problem-solving, and organizational skills.
• Valid driver’s license and ability to travel as needed.

Work Environment & Expectations:

• Must be willing to travel within designated service areas, including home visits and community meetings.
• Ability to work flexible hours, including evenings, based on family needs.
• Maintain confidentiality and adhere to HIPAA and agency policies.
• Participate in professional development opportunities as required.

Salary: $50,000-$65,000
