Position Summary

As a Float Pool LPN, you will conduct supervisory visits with clients receiving personal care assistance and provide skilled nursing services. This position requires travel to patient homes and the ability to work autonomously.
Responsibilities
- Provide supervisory visits or skilled nursing in the client’s home for clients receiving personal care assistance within the assigned coverage area.
- Be flexible and available to travel to different locations within the agency’s designated service areas, depending on patient needs and staffing shortages.
- Monitor and supervise the care of patients receiving homecare services to ensure compliance with care plans.
- Conduct supervisory assessments, including direct observation of Personal Care Support (PCS) personnel delivering care.
- Complete monthly supervisory assessments to maintain compliance with state and agency requirements.
- Maintain accurate and timely documentation of patient records in compliance with state regulations.
- Travel to client homes across the designated service region, ensuring coverage in high-need areas.
- Adhere to the Float Pool shift commitment, signing up for a minimum of one shift per week and being available for last-minute coverage needs when possible.
- Provide assistance with initial and annual skills training of Personal Care Assistants (PCAs).
- Ensure strict patient confidentiality in compliance with HIPAA regulations.
- Make informed decisions based on established agency policies and procedures.
- Prepare accurate and timely clinical progress notes.
- Inform families and the Registered Nurse of any changes in patient condition or care needs.
- Work independently with minimal assistance from the Clinical Manager.
- Adhere to the Georgia Licensed Practical Nurse Practice Act for all nursing services.
- Commit to signing up for at least one shift per week via the shared Float Pool calendar.
- Agree to the cancellation policy, which requires notification by 8:00 AM if the shift is not needed.
- Use the shared Float Pool scheduling platform for shift selection and tracking.
- $30 per hour (Float Pool LPN Rate).
- Active, unencumbered LPN license in the state of Georgia.
- Current BLS Certification required.
- Completion of the onboarding nursing quiz as part of the hiring process.
- Experience caring for medically fragile individuals is preferred.
- Home health experience is preferred but not required.
- Knowledge of policies and procedures for ICWP, CCSP, SOURCE, and GAPP programs is a plus.
