We are seeking a skilled Patient Care Manager - RN to join our team at Floyd HomeCare of Cedartown. As a Patient Care Manager, you will play a crucial role in the supervision and coordination of clinical services for our patients.
Key Responsibilities:
- Provide clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the health care team.
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
