We are seeking a Patient Care Manager - RN to join our team. As a Registered Nurse, you will work closely with home health patients and their families to provide high-quality care.
About the Role
- Provide clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with patient plans of care and in coordination with other healthcare team members.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Evaluate patient needs and ensure care rendered is individualized to meet those needs.
Requirements
- Current RN licensure in the state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
