POSITION SUMMARY:
Working under the direction of a Clinical Manager, the person in this position coordinates with and supports the clinical field staff. This position maintains high standards of care and facilitates a smooth and supportive experience for patients and their families.
RESPONSIBILITIES:
- Coordinates various aspects of care such as orders, patient triage, reviewing and approving supply orders, admission management and follow-up, and assigning labs as directed by the Clinical Manager.
- Provides clinical education, including mandatory requirements (in the field and off the field) .
- Actively involved in the daily operations and interacts with front-line clinicians on a routine basis.
- Supports the team by gathering data, facilitating and documenting case conferences.
- Reviews referrals prior to acceptance and triage, as needed.
- Participate in quarterly QAPI meetings and initiatives to incorporate clinical record reviews, trends & surveys.
- Contribute to monthly in-service and educational meetings with the clinical team in compliance with Medicare regulations.
- Review weekly reporting of delayed charting, proper care and documentation of wound care, high-risk patient needs, and reports to the Clinical Manager for direction.
- Assists with the orientation and precepting of new staff
- Conducts EMR chart audits, as needed
- Other duties as assigned
QUALIFICATIONS:
- Current and unrestricted CA LVN License
- Home Health Experience preferred
- A minimum of one year of LVN experience in a direct patient care setting
- The jobholder in this position must demonstrate current competencies applicable to the job description.
