Occupational Therapist (OT)
POSITION SUMMARY :
As a Home Health Occupational Therapist, you will evaluate and treat patients with injuries, illnesses, or disabilities in their homes. You will help patients recover, improve, and maintain skills needed for daily living.
RESPONSIBILITIES :
- Evaluate patient functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.
- Evaluate home environment for hazards or barriers to more independent living
- Identify equipment needs
- Provide supervision to the Occupational Therapy Assistants
- Conduct an initial evaluation / assessment and report findings to the Health Care Provider and RN and develops the plan of care promptly for patients
- Document initial evaluation and treatment plan and re-evaluations of patient’s progress at least every thirty (30) days and update plan of care as necessary.
- Complete comprehensive assessments and OASIS data, as indicated on a timely basis.
- Observe, record and report to Health Care Provider the patient’s treatment, response and progress and promptly report significant changes
- Obtain orders to revise plan of care
- Instruct patient / CG in the use of alternate methods and devices for upgrading independence in activities of daily living
