Focus Inc. is seeking a dedicated and resourceful Case Manager to join our Mental Health Skill-Building (MHSB) team. This role is ideal for those who are passionate about providing comprehensive support to clients with mental health challenges. The Case Manager will play a critical role in coordinating services, developing individualized service plans, and assisting with client growth within a supportive and structured environment.
Primary Responsibilities:
- Collaborating with community partners, healthcare providers, and support networks to coordinate comprehensive services.
- Responding promptly to client needs, providing crisis intervention when necessary.
- Managing client schedules and appointments using digital tools to ensure efficiency and accessibility.
- Facilitating intake assessments to determine eligibility and service requirements.
- Maintaining accurate and up-to-date documentation, including progress notes, consent forms, and compliance-related records.
- Assisting clients in accessing community resources, such as housing, employment support, and healthcare services.
- Advocating for clients' needs within the community and with service providers to ensure holistic care.
- Assessment of the individual’s needs that can be met by the required activities of the Qualified Mental Health Professional (QMHP).
- Development of an Individualized Service Plan (ISP) based on the problems and needs identified during the assessment and agreed upon by the provider and individual.
- Reviewing the ISP with the individual at least quarterly to ensure goals are relevant and progress is tracked.
- Ongoing revisions of the ISP as treatment needs change, with a comprehensive review and update completed annually.
- Provision of training and reinforcement of functional skills and appropriate behaviors related to health and safety, activities of daily living, and effective use of community resources.
- Assistance with medication management, including monitoring adherence and educating clients on proper usage.
- Monitoring clients' health, nutrition, and physical condition to support overall well-being.
- Maintaining timely and accurate documentation of all service provisions, ensuring compliance with regulatory standards.
Qualifications:
- Bachelor's degree in social work, human services, or a related field (preferred).
- Certification as a Qualified Mental Health Professional (QMHP) in Virginia (preferred).
- Minimum of two years' experience in case management, mental health support, or a related role.
- Strong organizational skills with the ability to manage multiple priorities effectively.
- Excellent communication and interpersonal skills, with a client-centered approach.
- Ability to work independently while thriving in a team-oriented environment.
- Valid driver’s license (required).
Minimum Knowledge, Skills, and Abilities:
- Knowledge of principles, techniques, and best practices related to community-based mental health support services.
- Familiarity with casework documentation requirements to ensure regulatory compliance.
- Understanding of mental health conditions and therapeutic interventions.
- Knowledge of community resources and entitlement programs.
- Ability to develop and implement effective Individual Service Plans.
- Strong collaboration skills for interacting with referral sources and community service providers.
- Ability to utilize clinical supervision for continuous improvement in service delivery and documentation.
