Key Responsibilities:
- Prepare comprehensive social histories for patients, assess their needs, and provide individualized social casework to promote optimal health outcomes.

- Collaborate with healthcare providers to understand significant social and emotional factors impacting patient care.
Requirements
- Licensed Clinical Social Worker (LCSW) credential required for most travel/contract positions.
- Independent decision-making skills and excellent communication abilities.
- Master's degree from a CSWE-accredited School of Social Work preferred.
- At least 2 years of experience in social work or a related field.
